UnitedHealth Group (UNH) — the variant view
The market prices UnitedHealth as if a 4% net margin is permanent; at its 2023 margin the same revenue would earn about $26 billion.
📈 UNH valuation, revenue & earnings — P/E, P/S, revenue, EPS →UnitedHealth's shares have recovered part of their fall but price the business at a fraction of its old profitability. At $376.59 on 25 September 2026 the company was worth $338.03 billion12, against $495.37 billion at the end of 2022 and $299.03 billion at the end of 20253. It traded at 24.24 times trailing earnings and 17.70 times forward earnings, and at 0.75 times sales4.
The price-to-sales ratio is the useful one for an insurer, because revenue is large and margins thin. Net margin was 6.0% in 2023, 3.6% in 2024 and 2.7% in 20255. The 2026 guidance of more than $16,750 million of net earnings6 on revenue of more than $439.0 billion7 implies about 3.8%8.
What the market is not pricing is a return to the old margin. At 2023's 6.0% net margin, the guided $439.0 billion of 2026 revenue would earn about $26.3 billion9, and today's market value would be about 12.8 times those earnings10. Analysts' average target of $481.7211 is higher than the current price but well below that arithmetic.
There are good reasons for caution. The Justice Department inquiries have no disclosed amount12, Medicare rates remain below the cost trend13, and Berkshire Hathaway bought about 5 million shares in 2025 and sold its entire stake in the first quarter of 20261415.
The shares have moved a long way in a short time. The 52-week range is $255.97 to $461.6216, and a second source puts the forward multiple at 16.64 rather than 17.7017. At 30 June 2025, before the worst of the earnings news, the shares were $311.9718. Institutions own about 89% of the stock and insiders less than 1%19, so there is no controlling holder whose patience could be counted on.
The market's scepticism has a precedent in the company's own record. Adjusted earnings per share fell from $27.66 in 2024 to $16.35 in 20252021, and management has restated a long-term earnings growth objective of 13 to 16 percent22. Investors are not yet paying for that objective: a forward multiple near 18 is below the roughly 19 to 22 times at which the shares ended each year from 2015 to 20202324.
The variant view is that the 2025 margin was an error, not a new normal. The number that decides it is the net margin in 2027: back above 5% would make today's price look low; stuck near 3.8% would make it fair.
