Counted by what outsiders pay, UnitedHealth is 77% insurer; counted by profit in 2025, the pharmacy business earned three-quarters as much as the insurer.
UnitedHealth reports four segments: UnitedHealthcare, Optum Health, Optum Insight and Optum Rx1. Their reported revenues add up to far more than the company's total, because Optum sells most of its output to UnitedHealthcare. In 2025 the segments' revenues included $167,956 million of sales inside the group, which are eliminated on consolidation2. About 62% of Optum's revenue came from inside the group3.

Seen this way, UnitedHealth is an insurer with services attached: about 77% of revenue is UnitedHealthcare's7. Seen by profit, it is more balanced. In 2025 UnitedHealthcare earned $9,425 million, Optum Rx $7,193 million, Optum Insight $2,624 million and Optum Health lost $278 million8.
The lines are moving apart in 2026. UnitedHealthcare and Optum Health are shrinking deliberately, while Optum Insight's margin has recovered to 25.3%9.
The internal sales are not a small adjustment. Corporate eliminations were $136,373 million in 2023, $150,887 million in 2024 and $167,956 million in 202510, and the Optum segments also sold $5,480 million to each other in 202511. Counted gross, the four segments reported $621,003 million of revenue in 2025 against the $447,567 million the company actually took in from outside12. Any comparison of Optum's size with a stand-alone competitor has to start from the external figures.
The resegmentation moves money between two of the four lines. On the 2026 basis, Optum Health's 2025 revenue was $100,051 million rather than $101,957 million, and Optum Insight's $21,034 million rather than $19,417 million13; UnitedHealthcare and Optum Rx were unchanged14. The external revenue in this app's chart follows the old basis for all five years so that the series is consistent.
The share of Optum's revenue that comes from inside the group is the figure to follow, about 60% in the second quarter of 202615: a falling share with rising outside revenue would mean Optum's businesses win on their own; a rising one would mean they mostly serve the insurer.
