When Change Healthcare went down in 2024, UnitedHealth lent care providers $9 billion interest-free to keep them paid.

Change Healthcare is the part of Optum Insight that connects providers and payers, and its importance was proved by its absence. On 21 February 2024 UnitedHealth disclosed that "a suspected nation-state associated cyber security threat actor" had gained access to some of Change Healthcare's systems1. The disruption interrupted the flow of payments to care providers.

The response showed what the system depends on. UnitedHealth lent providers $9,033 million interest-free in 20242, and recovered $4,514 million that year and $1,680 million in 20253. The attack affected approximately 190 million individuals4. The first-quarter 2024 impact alone was $0.74 a share5, and in the fourth quarter of 2025 the company booked a final $799 million charge related to collecting the provider loans6.

The episode is evidence of a moat and of its cost. A business whose outage forced its owner to lend providers $9 billion is plainly essential to them; it is also a single point of failure that regulators and customers now watch.

About $2.8 billion of the provider loans had not been repaid by the end of 20257, before the final charge.

The accounting for the episode ended with the 2025 results. The final $799 million charge in the fourth quarter of 20258 was part of $2,878 million of pre-tax items in that quarter, offset in part by a $442 million net gain on portfolio divestitures9.

Watch Optum Insight's external revenue and backlog. If outside customers keep using Change Healthcare's network, the switching costs survived the attack; if the external backlog keeps shrinking, some of them used it as a reason to leave.
