Optum Health's recovery is being bought by shrinking. The company said its 2026 revenue reflects "the strategic right-sizing of Optum Health"1, and in the second quarter of 2026 Optum Health revenue fell 5% year over year because of about 700,000 fewer value-based care patients2.

The patient counts show the trend. Optum Health served about 103 million people at the end of 2023, 100 million in 2024 and 95 million in 2025 on the old basis34; on the recast basis it served 92 million at the end of 2025 and 93 million in June 202656.

The first half of 2026 shows the trade. Optum Health revenue was $47,581 million against $49,562 million, down about 4%78, while operating earnings rose to $2,331 million from $1,840 million, about 27%910.

Shrinking the book is the right move for a business that was losing money on some contracts. It also concedes that the value-based model did not work at the scale UnitedHealth had built.

The January outlook had made the plan explicit, attributing Optum's expected revenue of more than $257.5 billion partly to membership attrition at Optum Rx and the strategic right-sizing of Optum Health11. The 2026 results so far are consistent with that plan.

Revenue and margin have to be read together. If revenue stabilises while the margin keeps rising, the business has found its right size; if revenue keeps falling, Optum Health is exiting its own strategy.
