UnitedHealth Group (UNH) — the variant view

In the second quarter of 2026 UnitedHealth earned 55% more on flat revenue, because it priced its insurance for the costs it had missed in 2025.

UnitedHealth's second quarter of 2026 was the clearest sign yet that the repair is working. Revenue was $112,032 million, barely changed from $111,616 million, but operating earnings rose to $7,991 million from $5,150 million1, about 55%2. Diluted earnings per share were $6.04 against $3.74, and adjusted earnings $6.38 against $4.083.

The insurer and the clinics drove the rebound.

The improvement came from pricing. The medical care ratio was 86.7% against 89.4%4, helped by $860 million of favorable development from earlier periods5. The operating cost ratio rose to 12.7% from 12.3%6.

Every segment earned more. UnitedHealthcare earned $3,942 million against $2,075 million, Optum Health $1,190 million against $429 million, Optum Insight $1,369 million against $1,205 million and Optum Rx $1,490 million against $1,441 million7.

The company is smaller. Medical membership was 48,525,000 in June 20268, and Medicare Advantage seniors served had contracted by 965,000 since year-end9. Cash from operations was $11.1 billion in the quarter10, and debt-to-capital fell to 41.2% from 44.1%11.

Management raised its 2026 outlook to $18.45 to $18.95 a share on a GAAP basis and $19.50 to $20.00 adjusted12, and in September management said it planned to reaffirm the adjusted range at an investor conference13.

The first quarter had set the pace. Revenue was $111,721 million, operating earnings $8,990 million and diluted earnings $6.90 a share, $7.23 adjusted, with a medical care ratio of 83.9%14. Six-month operating earnings of $16,981 million were about two-thirds of the full-year guidance1516, and six-month cash from operations was $19,964 million17.

There were one-off items on both sides. The half included a $400 million contribution to the United Health Foundation funded by a cash gain on an Optum Insight business disposition, a $187 million decrease in loss-contract reserves, and $133 million of losses on the South American operations held for sale18. The company also completed the acquisition of Alegeus on 2 July 202619.

The quarter confirms the recovery and its sources. What to check next is the third-quarter medical care ratio, due with results on 13 October20: a figure near 88% without large favorable development would say the repricing holds on its own.
