UnitedHealth Group (UNH) — threat to the moat
Sixty-two percent of Optum Health's revenue comes from UnitedHealthcare, so the clinics shrink whenever the insurer does.
Optum Health's revenue depends on its own parent. In 2025, $63,615 million of its $101,957 million of revenue came from affiliated customers1, about 62%2. Its external revenue, from other insurers and patients, was about $38.3 billion3.
That makes the clinics hostage to UnitedHealthcare's membership decisions. When the insurer repriced Medicare Advantage and lost about 880,000 members between December 2025 and June 2026456, Optum Health's affiliated revenue fell with it7.
The dependence also limits how the business can be valued. Its prices to UnitedHealthcare are set at management's estimate of fair value8.
The pattern goes back years. Optum Health's affiliated revenue was $29,234 million in 20219 and $63,615 million in 202510, more than doubling, while its external revenue grew from about $24.8 billion to $38.3 billion11. The internal business grew faster, which is why the dependence deepened just as the insurer's Medicare costs rose.
External revenue is the figure that matters. If it grows while affiliated revenue shrinks, Optum Health has a business of its own; if both fall, it is a department of the insurer.
