Optum owns the pharmacy manager, the claims pipes and the clinics its own insurer buys from, and 62% of its revenue comes from that insurer.

Optum is what makes UnitedHealth different from other insurers: it owns the pharmacy benefit manager, the claims technology and the clinics that its insurer buys from. The three Optum segments had revenue of $270,620 million in 2025, of which $167,956 million came from within the group1, about 62%2.

The pharmacy business carried Optum in 2025.

The businesses are very different. Optum Rx had revenue of $154,726 million at a 4.6% margin, Optum Insight $19,417 million at 13.5%, and Optum Health $101,957 million at a small loss on the old segment basis3. Optum's combined operating earnings fell from $16,703 million in 2024 to $9,539 million in 20254, about 43%5.

The integration gives Optum scale no stand-alone competitor could build as fast: a customer with about 49.8 million medical members6 whose prescriptions, claims and some of whose doctors run through Optum. It also sells to outsiders; the 10-K says its affiliated physician organizations "contract with competitors of UnitedHealthcare"7.

The limits are equally clear. Internal prices are set by management8, the IRS is questioning them9, and when UnitedHealthcare shrinks, Optum shrinks with it. In the second quarter of 2026 Optum earned $4,049 million, a 6.2% margin, against $3,075 million and 4.6% a year earlier10.

The Optum businesses also invest differently. In 2025 Optum Health spent $1,237 million on property, equipment and software, Optum Insight $1,170 million and Optum Rx $399 million, against $816 million at UnitedHealthcare11. Optum Insight's depreciation and amortisation of $1,422 million was the largest of any segment12, reflecting the software and acquired intangibles it carries.

The 2026 guidance expects much of the lost ground back. Optum is guided to more than $13,450 million of operating earnings in 202613, against $9,539 million in 202514, an increase of about 41%15; the realignment of Optum Financial does not change the Optum total16. In the first half Optum earned $7,345 million17.

This is a narrow moat: real scale, a captive first customer, and some genuine outside franchises, but a business whose profitability is hard to judge because most of its prices are set inside the family. The test is external revenue: if the part of Optum sold to outsiders keeps growing, the moat is Optum's; if it shrinks, the moat is simply UnitedHealthcare's size.
