UnitedHealth Group (UNH) — the future bets

UnitedHealth's bets are about repairing earnings, not finding new markets, and the first half of 2026 delivered two-thirds of the year's guided profit.

UnitedHealth's bets for the next few years are mostly about repair rather than expansion. After 2025's collapse, the company guided in January 2026 to revenue above $439.0 billion, a 2% decline reflecting "planned right-sizing across the enterprise"1. It has since raised its 2026 outlook twice, to adjusted earnings of $19.50 to $20.00 a share23, a midpoint of $19.754, against $16.35 in 20255.

Raised twice in six months.

The first bet is the deliberate shrinking of Medicare Advantage: about 880,000 fewer members between December 2025 and June 2026678. The second is leadership: Stephen Hemsley returned as chief executive in May 20259 with a one-time $60 million stock option award10. The third is Optum Health's margin, which the chief financial officer expects to reach about 6% by 202811. The fourth is a set of public commitments on prior approvals, drug rebates and exchange profits12.

None of these is a new market. They are attempts to restore the old earnings power: the company restated a long-term earnings growth objective of 13 to 16 percent13.

The first half of 2026 went well. Operating earnings of $16,981 million were about two-thirds of the full-year guidance of more than $25,450 million1415.

The bets also have a cost in capital returns. The January 2026 outlook assumed share repurchases of about $2,500 million16; by July the company expected at least $5,000 million17, and it had repurchased $4.0 billion of stock through mid-July 202618. The cash to do it comes from a business guided to generate about $24,000 million of operating cash flow in 202619, against $19,697 million in 202520.

The most important number for all four bets is the medical care ratio, because it decides whether shrinking, new leadership and repricing produce profits. The ratio was 83.9% in the first quarter of 2026 and 86.7% in the second2122, against full-year guidance of 88.1%23, which implies a higher ratio in the second half.

The bets will be judged on 2026 delivery. The guidance midpoint is the figure to watch: holding or rising through the October results would say the repair is working; a cut would say the 2025 problems were not fully understood.
