Membership has fallen for two and a half years: from 7,845,000 at the end of 2023 to 6,780,000 in June 202634, about 14%5.

The members who leave when eligibility tightens tend to be healthier than those who stay, so the remaining book costs more per person. A rate set on last year's population can be too low for this year's.

The shrinking accelerated in 2026. Medicaid membership was 7,380,000 at the end of 2025, 7,160,000 in March 2026 and 6,780,000 in June67, a loss of 600,000 in six months8, more than the previous two years combined.

The figure that tests this is the Medicaid membership count against revenue. If revenue keeps rising while membership falls, states are paying for the sicker mix; if both fall, the business is simply getting smaller at the states' direction.
