UnitedHealth holds $39 billion of claims not yet paid, and in June 2026 it held them about three days longer than at the start of the year.
Like any insurer, UnitedHealth collects premiums before it pays claims, and the gap is money it holds. Medical costs payable were $39,337 million at the end of 2025, up from $34,224 million1. Days claims payable, a measure of how long that money stays, was 44.1 at the end of 20252 and 47.0 in June 20263.
The float shows up in cash flow. Cash from operations was $29,068 million in 2023, $24,204 million in 2024 and $19,697 million in 20254, falling as profits did. In the first half of 2026 it was $19,964 million, against $12,644 million a year earlier5, and management guides to about $24,000 million for the year6.
The float is real but not free. Most of it sits in regulated insurance subsidiaries that must hold capital, and the company's investment income depends on interest rates.
The float also shows in the cash flow's timing. Cash from operations was $11.1 billion in the second quarter of 2026 alone, 1.9 times net earnings, which the company attributed partly to the timing of a substantial government payment7. The 2026 guidance for cash from operations of about $24,000 million compares with an original January figure of more than $18,000 million89.
This is a narrow advantage: every large insurer has float, and UnitedHealth's is large because its premiums are. Days claims payable is the figure to watch; a steady or rising number supports the reserves, while a sharp fall would suggest claims are being paid faster than estimated, or that reserves were thin.
