UnitedHealth Group (UNH) — the future bets

UnitedHealthcare gave up about 880,000 Medicare Advantage members in six months to restore a margin that had fallen to 2.7%.

The largest bet in UnitedHealth's plan is to get smaller where it was losing money. Medicare Advantage membership was 8,445,000 at the end of 2025 and 7,565,000 in June 202612, a fall of about 10%3. Including programs for people eligible for both Medicare and Medicaid, the company said seniors served had contracted by 965,000 since year-end4.

Growth reversed by choice.

The shrinking was planned. The January 2026 outlook projected UnitedHealthcare membership of 46.9 million to 47.5 million5 and revenue down 2% on "planned right-sizing"6.

The logic is that an insurer's scale is only valuable if the members are priced correctly. After a year in which the medical care ratio reached 89.1%7, keeping unprofitable members would have been worse than losing them.

The early results support it. UnitedHealthcare earned $3,942 million in the second quarter of 2026, a 4.6% margin against 2.4%8.

The shrinking extends to the rest of UnitedHealthcare. Total medical membership was 49,760,000 at the end of 2025 and 48,525,000 in June 2026910, and Medicaid alone fell by 600,000 over the same six months11. The company guided to UnitedHealthcare revenue of more than $335.0 billion for 202612, below 2025's $344,903 million13.

The risk is that repricing loses good members as well as bad. Medicare Advantage membership is the figure to watch: stabilising near 7.5 million while the margin rises would say the right members stayed; further losses without margin gains would say the company priced itself out of its own market.
