Optum Health was built to profit from managing costs and instead lost money when costs rose, because it carried the same risk as the insurer.
Optum Health was meant to be UnitedHealth's answer to rising medical costs: own the doctors, take a fixed payment per patient, and keep the savings from better care. In 2024 it earned $6,902 million on the 2026 segment basis; in 2025 it lost $1,115 million1. On the old basis its revenue was $101,957 million in 2025, and about 62% of it came from UnitedHealthcare23.
The model failed in 2025 for a simple reason. Under value-based arrangements the clinics bear medical cost risk, so when costs rose they lost money at the same time as the insurer. A report attributed the negative margins to rising costs and federal policies that restricted payments4.
The response has been to shrink and reprice. The company cited the strategic right-sizing of Optum Health in its 2026 outlook5, served about 700,000 fewer value-based patients in the second quarter6, and sold part of its Florida clinics to a private-equity partner7. Earnings have come back: $1,190 million in the second quarter, a 5.1% margin8.
The chief financial officer expects margins of about 2% in 2026, 4% in 2027 and 6% in 20289.
The leadership changed with the strategy. Patrick Conway became chief executive of Optum in May 202510, and Krista Nelson leads Optum Health11. The segment's capital base is large: $100,991 million of assets at the end of 2025 on the old basis12, much of it goodwill from the practices, clinics and home-health businesses UnitedHealth bought.
This is a thin moat. Owning clinics gives local strength and data, but the business carries the same risk as the insurer and competes for doctors with everyone else. The margin path is the test: reaching 4% in 2027 would say the model has been repaired; falling short would say value-based care at this scale does not pay.
