UnitedHealth Group (UNH) — moat facet

For most of a decade UnitedHealthcare paid out 82 cents of each premium dollar in claims; in 2025 it paid 89, and each point is worth $3.5 billion.

The best evidence that UnitedHealthcare knew how to price risk is how little its medical care ratio moved for most of a decade. The ratio is the share of premiums paid out as medical costs. It was about 81.7% in 2015, 81.2% in 2016, 82.1% in 2017, 81.6% in 2018 and 82.5% in 20191, then 79.1% in 2020, 82.6% in 2021 and 82.0% in 20222. An insurer that lands within a point of its target year after year, on premiums that grew from $127 billion to $257 billion3, has a pricing machine, not luck.

Flat for years, then up seven points in three.

The machine then slipped. The ratio rose to 83.2% in 2023, 85.5% in 2024 and 89.1% in 20254. Within 2025 it went from 84.8% in the first quarter5 to 89.4% in the second6 and 89.9% in the third7.

The arithmetic explains why this one number matters so much. Premiums were $352,229 million in 20258, so each point of the ratio is worth about $3.5 billion9, close to a fifth of that year's $18,964 million of operating earnings10. A ratio seven points above the old norm swallowed most of a year's profit.

The recovery has begun. The ratio was 83.9% in the first quarter of 202611 and 86.7% in the second, against 89.4% a year earlier12, and management guides to 88.1% for the full year13.

The discipline was also visible in the other ratio that matters, operating costs. The operating cost ratio was 14.7% in 2023, 13.2% in 2024 and 13.3% in 202514, and 12.7% in the second quarter of 202615. Because overhead held steady while medical costs rose, the whole deterioration in margin came from claims, which is why pricing, not efficiency, is the lever that has to work.

This is the heart of the insurer's moat, and it is intact but unproven. The test is simple: a full-year ratio at or below the 88.1% guidance would say the pricing discipline is back; another year near 89% would say the 2025 miss was not a one-off.
