Verdict: Merck owns the most successful cancer drug of its era and has told investors exactly when it stops being protected. Keytruda is half of sales and earns very high margins; Gardasil's China business has collapsed; Animal Health is steady but small. Merck is buying its replacement through expensive acquisitions charged straight to earnings, which is why trailing profit looks tiny. At about 17 times forward earnings the shares are fairly priced for a strong franchise with a known end, and cheap only if the launches and acquisitions grow several times over by 2029.

Merck makes prescription medicines, vaccines and animal health products, and at the moment it is mostly one cancer drug. Merck & Co., based in Rahway, New Jersey, is known as MSD outside the United States and Canada1. It employed about 75,000 people at the end of 20252 and reports two segments, Pharmaceutical, which includes human vaccines, and Animal Health3.

Sales were $65,011 million in 20254. Keytruda, an anti-PD-1 cancer antibody, sold $31,641 million, and Keytruda alone represented 49% of total sales5. Gardasil, the HPV vaccine, sold $5,233 million, down 39% after shipments to China stopped6. The rest of the Pharmaceutical segment, from Winrevair to the fading diabetes drugs, sold about $21,228 million7, and Animal Health $6,354 million8. About 56% of sales were American9.

How it makes money is the patented-medicine model: expensive research, long patents and very high product margins while they last. The Pharmaceutical segment earned $45,754 million of segment profit in 202510, before $14,987 million of central research that Merck does not allocate11. Net income attributable to Merck was $18,254 million, or $7.28 a share12.

Merck's reported earnings swing with its acquisitions, because it charges most of the price of acquired research straight to expense. Net income was $365 million in 2023, the year of the Prometheus and Daiichi Sankyo deals13, and the first half of 2026 was a net loss of $5,575 million after the Cidara and Terns charges14. Operating cash flow was $16,472 million in 202515, and Merck returned $13.3 billion to shareholders16.

In the second quarter of 2026 sales rose 5% to $16,607 million17. The shares closed at $147.98 on 24 September 2026, a market value of $365.09 billion18, 116.26 times trailing and 17.19 times forward earnings19.

The company has shrunk and regrown once already. In June 2021 it spun off its women's health, biosimilars and established brands businesses as Organon20; on the continuing basis its 2019 and 2020 sales were $39,121 million and $41,518 million, against $46,840 million and $47,994 million as originally reported21. Since then, sales have grown by about a third, from $48,704 million in 2021 to $65,011 million in 202522, and Keytruda supplied close to $14.5 billion of that increase23.

Merck's share count has been falling slowly. It had 2,472,392,003 shares outstanding at 31 January 202624, after buying back about 59 million shares for $5.1 billion in 202525, and expects to repurchase about $3.0 billion in 2026 with $5.7 billion of authorisation left at 30 June26. The buybacks are modest beside the acquisitions, which are where Merck's cash now goes.

The moat is narrow and narrowing on a published calendar. Merck says biosimilar competition to Keytruda could begin in December 2028 and that a Medicare price effective in January 2029 will cause American Keytruda sales to decline materially27. The number that would change the verdict is the sales of the products that must replace it: Winrevair, Welireg, Ohtuvayre and Capvaxive sold about $1.25 billion together in the second quarter of 202628, and unless that figure is several times larger by 2029, Merck will meet the cliff still mostly a Keytruda company.

Switching costs are moderate: an oncologist using Keytruda across many cancers has little reason to change while it is protected, but biosimilars will be substitutable. Replication is hard because it takes years of trials to build a label as broad as Keytruda's and vaccine plants take years to qualify, though the Chinese HPV vaccine shows it can be done. Pricing power is middling: high today, with Medicare setting Keytruda's American price from 2029. Disruption resistance is moderate because the dates are known. Network effects are minimal. Durability sits in the narrow band because Merck's high returns rest on one molecule with a published end date.
