Merck's China sales fell from $6.8 billion to $1.9 billion in two years because one distributor stopped buying Gardasil.

Merck's second largest threat has already happened. Its sales in China were $6,802 million in 2023, $5,494 million in 2024 and $1,939 million in 20251, a fall of about 71%2. Gardasil, which had been the bulk of it, fell 39% worldwide in 20253.

Most of the fall came in one year.

The mechanism was a single channel. Merck sold its vaccines in China through one distributor, Zhifei4. Channel inventory built up, Merck paused shipments in February 2025, and in mid-2025 decided not to ship to China again that year5. It expects Gardasil sales in China "will not materially increase in 2026"6, and the limited shipments it began in the second quarter of 2026 are expected to be immaterial7.

The fall has legal consequences. A securities class action alleges that Merck made materially false and misleading statements about demand for Gardasil in China8; lead plaintiffs were appointed in December 2025 and an amended complaint was filed in February 20269.

It also has competitive ones. China approved its first domestic nine-valent HPV vaccine in May 202510, and the market Merck left is now available to it, as the Competitors page describes.

The rest of Merck's China business faces its own pressure. Merck reports that new entries to China's national reimbursement list averaged price reductions of approximately 60%, and that mature products in volume-based procurement took price cuts of more than 50%11. Its property, plant and equipment in China was $218 million at the end of 2025, against $15,021 million in the United States12, so the country was always a sales market rather than a manufacturing base.

The quarter shows no rebound yet. Merck's sales in China were $408 million in the second quarter of 2026 against $446 million a year earlier, and $797 million in the first half against $1,148 million13. The revised Zhifei contract has not changed the direction.

This threat is about the ceiling of the recovery rather than a further fall. Merck's China sales, $1,939 million in 202514, are the measure; if they have not returned above $3 billion by 2027, the old China vaccine business is gone for good and Gardasil will be a mainly American and European product.
