Each new European label brings Keytruda more patients at a lower price, and the patent there ends in 2031.
Outside the United States, Keytruda's breadth works against its price. Merck reported that the 2025 launch and reimbursement of new Keytruda indications in the European Union had a negative impact on pricing in those markets1. More indications meant more patients but a lower price per treatment.
International sales still grew. Keytruda sold $12,812 million outside the United States in 2025 against $11,610 million in 20242, about 10.4% more3. Volume is winning for now.
The limit is the European patent date. Merck expects to lose market exclusivity for Keytruda in Europe in 20314, and payers there know it. Health systems that negotiate on value have every reason to press price as the protected years shorten.
Europe is also where Merck's currency exposure sits. Merck estimates that a uniform 10% weakening of the dollar would have reduced income before taxes by about $131 million in 2025 and $239 million in 20245, small against its earnings because much of its foreign exposure is hedged. The European threat to Keytruda is therefore price and patents, not exchange rates.
The signal is European pricing disclosure in the annual report's Keytruda discussion. A second year of negative price effects with slower international growth would mean the label is being traded for price before the patent goes.
