Keytruda: One Molecule, Half the CompanyWide moat

Keytruda earns half of Merck's sales and its own maker has published the dates on which that stops.
Merck's moat today is a single antibody. Keytruda, with its new injectable form, sold $31,681 million in 202512, and Keytruda alone represented 49% of Merck's total sales3. In the second quarter of 2026 the franchise sold $8,366 million, up 5%4, about 50% of the quarter's sales5.
The advantages are real. Keytruda is approved in over 40 American indications across 19 tumour types6, so a rival must win cancer by cancer against the incumbent standard. Its largest royalty ends after 20267, lifting an already high margin. And Keytruda Qlex, the subcutaneous version, carries an American patent to 20438.
The limits are just as clear, because Merck published them. Biosimilar competition could begin in December 20289. Merck expects Keytruda to be selected for a Medicare negotiated price in 2027, effective 1 January 2029, and says that American sales "will decline materially after that time"10. Those two dates are the subject of the root threat on this page.
The concentration grew because Keytruda kept winning. It was about 28.3% of sales in 2019 on the continuing-operations basis, about 35.3% in 2021 and about 48.7% with Qlex in 202511. The franchise outgrew everything Merck owns except the problem it creates.
The segment's economics explain why the concentration is tolerated. The Pharmaceutical segment's own costs in 2025 were $6,679 million of cost of sales and $5,874 million of selling expense12 against $58,142 million of sales13. The research that created Keytruda and must replace it is booked outside the segment: $14,987 million of unallocated research in 2025, against $30,008 million in 202314, the year of the large acquisitions.
This is a wide advantage that is narrowing by the calendar. The number to hold it against is Qlex's share of the franchise, 5.5% in the second quarter of 202615: the moat survives 2028 only to the extent patients have moved to the protected form or to new medicines by then.
Keytruda's share rose to 49% while biosimilars (2028) and a Medicare price (2029) approach.
