Keytruda outsells Opdivo about three to one, and Bristol Myers Squibb still pays Merck royalties on another drug.
Bristol Myers Squibb is Merck's most direct competitor and one of its business partners. Its Opdivo is the other large anti-PD-1 antibody; it sold $10,049 million in 2025, up 8%1. Keytruda sold $31,641 million2, about 3.1 times as much3.
Both companies have launched subcutaneous forms to extend their franchises. BMS's Opdivo Qvantig sold $238 million in 20254; Keytruda Qlex, approved in September 2025, sold $463 million in the second quarter of 2026 alone5.
The partnership runs both ways. Merck receives royalties on Reblozyl, a BMS drug, and they were $525 million in 20256. Merck pays BMS a 22% royalty on Winrevair7, the pulmonary hypertension drug that came with Acceleron. Money flows in each direction every quarter.
The lesson is that in this industry a rival is rarely only a rival. Deals struck years ago over molecules neither company then sold at scale create lasting financial ties.
The Reblozyl stream is growing quickly. Merck's royalties on it were $17 million in 2021, $166 million in 2022, $212 million in 2023, $371 million in 2024 and $525 million in 202589. A rival's success in one disease is a source of income in Merck's accounts.
Keytruda's lead over Opdivo is the clearest evidence of Merck's competitive strength. The ratio of the two, about 3.1 in 202510, is the number to follow; a ratio falling toward 2 before 2028 would mean Opdivo's newer combinations are taking patients Keytruda used to win.
