Merck stops paying Keytruda's last big royalty after 2026, two years before copies can arrive in America.
Keytruda's margin is about to get a little wider for a reason unrelated to medicine. Merck paid a royalty of 6.5% on worldwide Keytruda sales to one third party through December 2023, which fell to 2.5% in 2024, and the royalty continues through 2026 before terminating1. A separate 2% royalty on American sales has already ended2.
At 2.5% of the $31,681 million the franchise sold in 2025, that royalty cost about $792 million a year3. From 2027 that money stays with Merck. The royalty also applies to Keytruda Qlex4, so the new injection inherits the same saving.
This is why the Pharmaceutical segment already earns so much. Segment profit was $45,754 million on sales of $58,142 million in 2025, a margin of about 78.7%56. That figure excludes central research, amortisation and restructuring, which Merck does not allocate to the segment7, so it measures what the products earn before the laboratory is paid for.
The timing is the irony. The royalty ends two years before biosimilar competition could begin in December 20288. Merck will enjoy its most profitable Keytruda years at exactly the moment the drug's American exclusivity is running out.
The saving matters because Keytruda's costs are otherwise small. Merck's cost of sales was $16,382 million in 2025 on sales of $65,011 million9, a gross profit of about $48,629 million10, or about 74.8%11. A royalty of about $792 million a year is roughly five per cent of that cost base12, enough to move the margin visibly when it ends.
The saving is real but temporary. The Pharmaceutical segment margin in the second quarter of 2026 was about 78.7% on the recast basis1314; a margin that fails to rise in 2027, when the royalty has gone, would say pricing or mix is already giving back what the contract released.
