Keytruda is approved in over forty American indications, so a rival must win cancer by cancer against the standard of care.

Keytruda is hard to displace because it is not one drug in one disease. It is an anti-PD-1 antibody that has been approved in over 40 indications in the United States, including 19 tumour types and 2 tumour-agnostic indications1. An oncologist who treats lung, melanoma, kidney, bladder or endometrial cancer has used it, and each new label adds patients without adding a new sales force.

Breadth a rival must match cancer by cancer.

The sales show what breadth is worth. Keytruda sold $17,186 million in 2021, $20,937 million in 2022, $25,011 million in 2023, $29,482 million in 2024 and $31,641 million in 202523. That is a compound growth rate of about 16.5% a year over four years4, on a base that was already the company's largest product.

It sells on both sides of the Atlantic. In 2025 American sales were $18,829 million and international sales $12,812 million5. The United States carries about 60% of it6, which matters later, because the American price is the one Washington will set.

Breadth also protects against a single trial failure. A new competitor has to win indication by indication, and each win takes years of trials against an incumbent that is already the standard of care in most of them.

The growth is slowing as the label fills up. Keytruda grew about 7.3% in 20257 and the franchise, including the new injection, grew 5% in the second quarter of 20268. Nothing about breadth protects the molecule after its patents end.

The newest labels are in earlier disease. Merck attributed Keytruda's second-quarter 2026 growth primarily to "strong global uptake in earlier-stage indications", naming triple-negative breast cancer, cervical, head and neck and bladder cancer9. Earlier-stage use means patients treated for longer and sooner, which is why the drug keeps growing even as its metastatic uses mature.

The verdict here is that Keytruda's breadth is a real, wide advantage with a fixed end date. Watch international sales: they grew faster than American sales in 2025, and a stall outside the United States would say the label has stopped expanding where it matters.
