Merck & Co. (MRK) — moat facet
Keytruda sold $31.7 billion in 2025 and nearly tripled in six years, and every year it grows the eventual fall gets larger.
Keytruda is the largest line in Merck's accounts and the reason the company is valued as it is. With Keytruda Qlex it sold $31,681 million in 202512, 48.7% of Merck's sales3; Keytruda alone was 49% by Merck's own rounding4. In the second quarter of 2026 the franchise sold $8,366 million, up 5%5.
What is inside the line is one molecule, pembrolizumab, sold in two forms. The original is an intravenous anti-PD-1 therapy approved in over 40 American indications, across 19 tumour types and 2 tumour-agnostic indications6. Keytruda Qlex is a subcutaneous version, approved by the FDA in September 20257, which sold $40 million in 20258 and $463 million in the second quarter of 20269.
The history is one of steady label expansion. On the continuing-operations basis Keytruda sold $11,084 million in 2019 and $14,380 million in 202010, then $17,186 million in 2021, $20,937 million in 2022, $25,011 million in 2023, $29,482 million in 2024 and $31,641 million in 20251112. It nearly tripled in six years13.
How it is paid: Merck sells its human medicines primarily to wholesalers and retailers, hospitals, government agencies and managed care organisations14. American sales were $18,829 million in 2025 and international sales $12,812 million15.
Profitability is not disclosed by product. The Pharmaceutical segment, of which Keytruda is more than half, earned a segment profit margin of about 78.7% in 202516, before central research, amortisation and restructuring17. The line pays a 2.5% royalty to one third party through 2026, after which it ends18.
Growth is slowing. Keytruda grew about 17.9% in 2024 and about 7.3% in 202519, and the franchise 5% in the second quarter of 202620, which Merck attributes mainly to uptake in earlier-stage cancers21. First-half sales included a favourable wholesaler timing effect of about $250 million22.
Keytruda also carries Merck's largest single royalty obligation in the other direction. Merck paid 6.5% on worldwide Keytruda sales to one third party through 2023 and 2.5% since23, and a separate 2% royalty on American sales ended in September 202424. From 2027 the franchise will carry no large royalty at all, which is why its profitability is likely to be highest in the two years just before its American patent expires.
The line's history before 2021 is on a different basis. On the continuing-operations basis restated after the Organon spin-off, Keytruda was $11,084 million of $39,121 million of sales in 2019, about 28.3%2526. Its share of the company has risen in every year since, not only because it grew but because the rest of Merck was reshaped around it.
The outlook has two dates. Biosimilars could begin in December 2028 and a Medicare price takes effect in January 2029, as the Moat and root threat pages explain27. The line's growth rate is what to watch until then; if the franchise is still growing at 5% or more in 2028 while Qlex has not passed a quarter of it, the eventual decline will start from the highest possible base.
Growth slowed from about 18% in 2024 to 7% in 2025 and 5% in Q2 2026.
