UnitedHealth Group (UNH) — moat facet

Forty-four percent of UnitedHealth's revenue comes from one customer, the federal government, and the only backlog it discloses is mostly its own.

UnitedHealth has one customer that matters above all others: the federal government. Premium revenue from the Centers for Medicare & Medicaid Services was 44% of consolidated revenue in 2025, against 40% in 2024 and 20231, 38% in 2022 and 36% in 20212. On 2025 revenue of $447,567 million3, 44% is about $197 billion4.

Rising every year but one.

Revenue concentration and backlog concentration give different answers. The insurer discloses no order backlog; its revenue comes mostly from premiums, and risk-based premiums are nearly 80% of consolidated revenue5. The only disclosed backlog is Optum Insight's, $31.1 billion at the end of 20256, and its largest customer is UnitedHealth itself: $12.9 billion of it was affiliated7, about 41%8.

Outside government, the customers are diffuse. The 10-K says concentrations of credit risk in receivables "are limited due to the large number of employer groups"9. Commercial membership was 29,920,000 in June 202610.

And inside the group, UnitedHealthcare is Optum's largest customer, buying $167,956 million of its services in 202511.

Government exposure runs beyond CMS. Community & State, which serves Medicaid members under contracts with 32 states and the District of Columbia12, had revenue of $94,390 million in 202513. Medicaid premiums are paid by the states, so the share of revenue that comes from governments is larger than the 44% CMS figure alone. The employer book, by contrast, is spread across a large number of groups14.

Revenue concentration has risen for five years. CMS premiums were 36% of revenue in 2021, 38% in 2022 and 40% in 202315, 40% in 2024 and 44% in 202516. The rise reflects the growth of Medicare Advantage, now being reversed in part by the deliberate shrinking of 2026.

The client base is broad in number and narrow in who writes the largest cheques. The CMS share is the figure to follow: a rise toward half of revenue would tie UnitedHealth's profits even more closely to one government's payment rules.
