Optum's earnings fell 43% in 2025, more than the insurer's, because its clinics carried the same medical cost risk.

The part of UnitedHealth meant to smooth the insurance cycle fell further than the insurer in 2025. Optum's operating earnings were $15,943 million in 2023, $16,703 million in 2024 and $9,539 million in 2025, and its margin fell from 7.0% to 6.6% to 3.5%1. The decline was about $7.2 billion2.

The services side fell the furthest.

Almost all of it came from Optum Health, whose earnings went from $7,770 million to a loss of $278 million on the old basis3. Optum Rx grew and Optum Insight fell by less.

The lesson is that vertical integration concentrates risk as well as spreading it. Optum Health's clinics carry medical cost risk under value-based contracts, so when medical costs rose, both the insurer and the clinics were hit at once.

The quarterly figures show that the fall accelerated through the year. On the 2026 basis Optum Health earned $1,411 million in the first quarter of 2025 and lost $2,996 million in the fourth4, while Optum Insight's earnings fell from $1,164 million to $172 million5. Only Optum Rx improved, from $1,318 million to $2,885 million6.

Optum's margin is the figure to read. A full-year margin back above 6% would say 2025 was an aberration; a year near 4% would say the integrated model is structurally less profitable than it looked.
