UnitedHealth Group (UNH) — moat facet

Optum Insight earns about $6.5 billion from outside customers at the highest margin in the group, and two-thirds of its revenue still comes from inside.

Optum Insight is the smallest segment and the most profitable per dollar. On the old basis it had revenue of $19,417 million in 2025, of which $12,948 million came from affiliates1; its revenue from outside customers was about $6.5 billion2, under 2% of the group's revenue3. That external figure is its band in the chart.

What is inside it is data and analytics, software, revenue-cycle and claims services, and Change Healthcare's network connecting providers and payers4. From 2026 it also includes Optum Financial and Optum Bank, with nearly 26 million consumer accounts and more than $27 billion of assets under management5. On that new basis its 2025 revenue was $21,034 million6.

The history is uneven. External revenue was about $4.3 billion in 2021, $5.3 billion in 2022, $8.0 billion in 2023, $6.9 billion in 2024 and $6.5 billion in 20257. The 2024 fall followed the Change Healthcare cyberattack of February 20248.

How it is paid: by contracts, often multi-year. Its backlog was about $31.1 billion at the end of 2025, $18.3 billion of it expected within twelve months9, and about 41% of the total was affiliated10.

Profitability is high but has fallen. Operating earnings were $3,398 million in 2021 and $3,588 million in 202211, $4,268 million in 2023, $3,097 million in 2024 and $2,624 million in 202512, a margin that fell from about 27.9% to 13.5%1314. On the 2026 basis the segment earned $1,369 million in the second quarter of 2026, a 25.3% margin15.

The outlook is guided at more than $4,925 million of operating earnings for 202616. Revenue from outside customers was about $1.8 billion in the second quarter17.

The quarterly pattern in 2025 showed the strain. On the 2026 basis Optum Insight earned $1,164 million in the first quarter, $1,205 million in the second, $920 million in the third and $172 million in the fourth18, before recovering to $963 million in the first quarter of 202619 and $1,369 million in the second20. Its segment assets rose to $62,875 million by June 202621 as Optum Financial and Optum Bank were added.

Its business depends on customers outside the group trusting a supplier owned by an insurer. The Change Healthcare attack of February 2024 affected approximately 190 million individuals22, and the external backlog fell from about $20.3 billion to $18.2 billion during 202523. How outside hospitals and insurers behave over the next few years will decide whether the segment's margin is earned in the market or on internal work.

The verdict turns on outside demand. It would change if external revenue kept falling while the margin rose, because Optum Insight would then be earning more by selling mainly to its own parent.
