AbbVie (ABBV) — threat to the moat
Most of AbbVie's cosmetics sales come from American and Chinese consumers paying cash, which makes the business move with their confidence.
Aesthetics is geographically concentrated. Management said the United States and China are a vast majority of the aesthetics business1. Both markets depend on consumers paying cash for elective treatments.
That makes the business cyclical in a way AbbVie's prescription drugs are not. The 10-K attributes the American Botox Cosmetic decline partly to decreased consumer demand2, and China adds regulatory and economic risk on top.
Botox Cosmetic sold $1,504 million in the United States and $1,098 million internationally in 20253.
China adds a separate risk. A cash-pay consumer market in a country with its own regulators and competitors can shift quickly, and Botox Cosmetic is approved in China for masseter muscle prominence4, a use that shows how the market there differs.
The concentration also limits diversification. An aesthetics business spread across many countries would smooth out a weak year in one; one that depends on two moves with their consumers. Management's own description of the business as mostly American and Chinese5 makes that plain.
Aesthetics growth in the United States is the tell. Another year of decline would mean the business is shrinking with its consumers, not just resetting prices.
