About $5 billion of J&J's medicine sales, in Simponi and Opsumit, faces copies right behind Stelara.

Stelara will not be the last expiry. The filing says at least two biosimilars are pursuing regulatory approval for a Simponi biosimilar in the United States, which would likely result in a significant reduction in future sales1, and that J&J expects generic competition for Opsumit in 20262.

The two carry real money. Simponi and Simponi Aria sold $2,668 million in 2025 and Opsumit and Opsynvi $2,325 million3, together about $5.0 billion4. Simponi is already slipping: second-quarter 2026 sales fell 10.5% to $618 million5.

Neither is Stelara's size, but they come as Stelara's decline is still running. The replacement machine has to cover overlapping losses rather than one at a time.

There is a third, smaller expiry in the same window. Remicade, an older biologic already facing biosimilar competition, fell 25.8% to $338 million in the second quarter of 20266. The filing's own description of Remicade's 2025 growth credited favourable patient mix and the return of European rights by Merck, partially offset by continued biosimilar competition7. Each of these products is small alone; together with Stelara they are a steady subtraction the growth products must overcome every quarter.

Opsumit, at least, is still growing on its way to the cliff: Opsumit and Opsynvi sold $1,208 million in the first half of 2026, up 9.4%8. That makes the eventual step down steeper, because the generic arrives against a larger base.

The test comes in 2027, the first full year with all three losses in the base, and it is Innovative Medicine operational growth. Holding above 5% would show the oncology and neuroscience growth is large enough to absorb a sequence of expiries.
