J&J's medicines got a tariff exemption in 2026; its devices, which cost far more to make, did not.

Devices are physical goods made in factories and shipped across borders, and that shows up in the cost line. MedTech's cost of products sold was $14,549 million in 2025, about 43% of its sales, against $15,646 million, about 26%, for Innovative Medicine12. The filing names tariffs, unfavourable transactional currency and macroeconomic factors as reasons MedTech's cost of products sold rose as a share of sales3.

Tariffs are a policy risk J&J cannot price its way out of. The company's January 2026 agreement with the United States government gave its pharmaceutical products an exemption from tariffs4; the announcement did not extend that to MedTech.

The risk factors add that the government may in the future pause, reimpose or increase tariffs5.

Currency adds to the same pressure. The filing names unfavourable transactional currency alongside tariffs in MedTech's cost of products sold6, and the company's 2026 guidance assumes a euro at $1.157. A device made in one currency and sold in another is exposed at both ends; a medicine whose value lies in its patent rather than its manufacturing is far less so.

The first-half figures show the pressure persisting. MedTech's cost of products sold was $7,438 million in the first half of 2026 against $6,964 million a year earlier8, up about 6.8%9, slightly faster than the segment's 6.0% sales growth10.

MedTech's gross margin is the gauge. If cost of products sold rises above 45% of segment sales, tariffs and currency are taking the margin the orthopaedics separation is supposed to add.
