Johnson & Johnson's largest product earns 15% of its revenue on patents Genmab owns, and pays about $2.4 billion a year for the privilege.
Darzalex is the single most important product Johnson & Johnson sells, and J&J does not own the patents that protect it. Darzalex and its under-the-skin version Darzalex Faspro accounted for approximately 15.0% of the company's total revenues in 20251, and the two patent families behind it belong to Genmab A/S, which licenses them exclusively to J&J's Janssen Biotech2.
That arrangement costs real money. The royalty runs from 12% to 20% of total Darzalex net sales, and J&J paid Genmab approximately $2.4 billion in 2025 and $2.0 billion in 20243. On sales of $14,351 million that is an effective royalty of about 16.7%4, a toll most owners of a blockbuster never pay. A reader should think of Darzalex as a product J&J rents on very good terms rather than one it owns outright.
The terms have been worth every cent. Worldwide sales went from $9,744 million in 2023 to $11,670 million in 2024 and $14,351 million in 20255, growth of 23.0% last year, which the filing puts down to continued share gains and market growth6. In the second quarter of 2026 the drug sold $4,207 million, up 18.9%7, and over the first half $8,171 million8. A product this size still growing near twenty percent is uncommon, and it is the main reason oncology passed $25 billion.
What J&J brings to the arrangement is not the molecule. It is the clinical programme that keeps moving the drug earlier in treatment, the subcutaneous formulation, and a commercial organisation that reaches haematologists in every major market. The 2025 pipeline table lists Darzalex filings in frontline transplant-ineligible myeloma and as monotherapy for high-risk smoldering myeloma9. Each new indication extends the time over which the royalty-bearing sales are earned, which is the practical form a moat takes when the patent belongs to someone else.
The royalty also takes a share of every extra Darzalex dollar. Genmab's rate ranges from 12% to 20% of total net sales10, and the filing does not say how the rate moves within that range. What it does show is that the royalty rose from $2.0 billion to $2.4 billion in a year in which Darzalex sales rose by about $2.7 billion1112, so roughly one dollar in seven of the increase went to the patent owner13.
Geography adds resilience. Darzalex sold $2,435 million in the United States and $1,772 million internationally in the second quarter of 2026, growing 20.7% and 16.4%14. A franchise growing in both halves of the world is not dependent on one payer system.
Darzalex is a superb business with a date on it. Follow its growth each quarter against the 18.9% of the latest one; a fall into single digits before 2029 would mean the franchise peaks before the patents do, which is the worst order in which those two things can happen.
Sales grew 23.0% in 2025 and 18.9% in the latest quarter as the drug moves into earlier lines.
