Johnson & Johnson (JNJ) — moat facet

J&J's three largest wholesalers handle nearly half its gross revenue, but the customers who matter are the payers who take $57 billion a year in rebates.

Johnson & Johnson does not name its largest customers, but it does disclose how concentrated they are. In 2025 three wholesalers distributing products for both segments represented approximately 21.8%, 15.5% and 11.1% of total gross revenues1, about 48% together2. In 2024 the three represented 20.5%, 15.6% and 12.3%3, and in 2023 18.2%, 15.1% and 14.2%4.

The routes and the products are concentrated; the buyers are not.

That concentration is less dangerous than it looks, because a wholesaler is a delivery route rather than a buyer with its own demand. Medicines move from J&J to retailers, wholesalers, distributors, hospitals and healthcare professionals5; the wholesaler holds the inventory for days, while the prescription is written by a physician and paid for by someone else.

The customers who decide J&J's economics are those who pay. Innovative Medicine accrued $56,819 million of rebates in 2025 against net sales of $60,401 million6, a ratio of about 94%7. The filing calls payors and pharmacy benefit managers a potent force in the marketplace8, and books the rebates, discounts and governmental clawbacks they extract as reductions in sales9. They never take delivery of a pill and they set the price of most of them.

Chief among those payers is the United States government. The United States was 57.1% of J&J's sales in 20251011, and three J&J drugs, Xarelto, Stelara and Imbruvica, were on Medicare's first selected-drug list with government-established prices from 202612. In January 2026 J&J agreed with the administration to provide Medicaid access at prices comparable with other developed countries13.

The other concentration to check is by product rather than customer. Darzalex was approximately 15.0% of revenue in 2025, Stelara 6.5% and Tremfya 5.5%14, about 27% from three products15. That concentration is larger than any single wholesaler's share and, unlike the wholesalers, it has an expiry date.

The distribution structure varies by segment. Medicines move through wholesalers to pharmacies and hospitals; devices are sold far more directly to hospitals and health systems, which negotiate prices and accrue rebates of their own, $6,446 million in 202516. The wholesaler concentration is reported for both segments together17, so the concentration in medicines alone may well be higher than 48%.

The geography is its own form of concentration. International markets bought $40,441 million in 20251819, and Europe alone $21,535 million20, but the United States is where prices are highest and where government pressure on them is rising fastest.

The concentration has barely moved over three years. The top three wholesalers took about 47.5% of gross revenues in 2023 and 48.4% in both 2024 and 2025212223, even as J&J's product mix changed a great deal. The routes to market are stable; what flows through them is not.

The wholesalers are paid for speed as well as volume. Innovative Medicine accrued $1,314 million of cash discounts in 202524, the price J&J pays for prompt payment by the distributors who carry its medicines. It is a small number next to the rebates, which is itself the point: the distributors take a thin slice for logistics, and the payers take the large one for access.

The verdict is that J&J's customer base is concentrated in its routes to market and in its payers, not in any buyer with discretion. The number to watch is the ratio of rebates to Innovative Medicine net sales: it rose from about 87% in 2023 to 94% in 2025252627, and a continued rise would mean the payers are taking a growing share of every dollar of list price.
