Amgen's largest line fell in 2026 for the first time, as Repatha and EVENITY overtook the denosumab drugs losing to copies.

General medicine and bone is Amgen's largest line, and it is two businesses moving in opposite directions. It sold $11,614 million in 20251, about 31.6% of total revenues2, from four drugs: Repatha for cholesterol, $3,016 million; EVENITY for osteoporosis, $2,100 million; and the two denosumab brands, Prolia $4,414 million and XGEVA $2,084 million3.

XGEVA appears here although Amgen's own releases list it under oncology, because it contains the same active ingredient as Prolia4 and is losing to the same biosimilars. Keeping denosumab in one band shows its erosion in one place.

The line's history is growth. It sold $5,457 million in 20195, and grew every year to 2025, about 2.1 times in six years6. Prolia was the engine for most of that time: $2,672 million in 2019 and a peak of $4,414 million in 202578.

It is paid for in the ordinary way, through insurers and Medicare after rebates, and Repatha's growth has come with falling net prices9. Amgen reports no profit by line; EVENITY's profits are shared equally with UCB10, so part of this band's revenue carries a partner's cost.

The turn came in 2026. In the second quarter the band sold $2,778 million, against $2,868 million a year earlier11, about 3% lower12, the first decline in the series. Inside it the growing half, Repatha and EVENITY, sold $1,667 million against $1,111 million for denosumab13; a year earlier the order was reversed, $1,214 million against $1,654 million14.

That crossover is the outlook. Repatha grew 37% and EVENITY 38% in the second quarter15, while Prolia and XGEVA fell 32% and 34%16.

Prolia is the band's history in one product. Launched in the United States and Europe in 201017, it grew every year from 2019 to $4,414 million in 20251819, then sold $727 million in the first quarter of 2026 and $759 million in the second2021. Repatha, launched in 201522, took a decade to pass $3 billion. The band's future depends on Repatha and EVENITY compounding faster than Prolia is now falling.

The verdict turns on when the band grows again. If its quarterly sales are back above the prior year by the second half of 2027, the growing drugs will have absorbed the copies; if the band is still shrinking then, denosumab's decline will have outlasted Repatha's and EVENITY's growth.
