Amgen (AMGN) — threat to the moat
Amgen's rebates tripled between 2019 and 2025 while its net sales grew by about half.
The rebate line is the fastest-growing number in Amgen's accounts. Rebates charged against product sales were $6,825 million in 2019 and $21,697 million in 202512, about 3.18 times3, while net product sales grew about 1.58 times4.
Rebates are what Amgen pays insurers and benefit managers for placement on formularies, and what it owes Medicaid and Medicare under statute. As more of its portfolio competes in crowded classes, psoriasis and cholesterol among them, it pays more to stay on the list. At the end of 2025 Amgen carried $10,606 million of accrued sales deductions on its balance sheet5.
The danger is that rebates are a ratchet. Once a benefit manager has been paid a larger rebate for a drug, lowering it risks losing the formulary position, and the next competing drug arrives offering more. Only a patent expiry or a new drug with no alternative resets the bargain.
Amgen's answer is products without direct competitors: TEPEZZA and KRYSTEXXA currently do not face any direct competitors in the United States, Europe or Japan, the 10-K says6. Those are exactly the drugs it bought with Horizon.
Watch the rebate line against product sales. If rebates again grow at twice the rate of net product sales in 2026, the rare-disease strategy is not offsetting the rest of the portfolio.
