McKesson, Cencora and Cardinal Health bought $59.8 billion of Amgen products at gross prices in 2025, more than one and a half times its net revenue.
Three companies carry almost everything Amgen sells in America. In 2025 McKesson bought $26,253 million of Amgen's products at gross prices, Cencora $20,986 million and Cardinal Health $12,562 million1, 34%, 27% and 16% of total gross revenues2. Together that is $59,801 million3.
The numbers look strange against Amgen's reported revenue until the gross-to-net arithmetic is remembered. Total revenues were $36,751 million4. McKesson alone bought gross product worth about 71% of Amgen's entire net revenue5; the three together bought about 1.63 times it6. The difference is the rebates and chargebacks that flow back after the wholesalers have sold the drugs on.
The concentration is stable. The three took 79% of gross revenues in 2023 and 77% in 2024 and 20257. It reflects the structure of American drug distribution, which Amgen did not choose and cannot change.
The risk is operational rather than commercial. A wholesaler cannot easily stop buying Amgen's drugs, because pharmacies and hospitals need them. But a failure in a wholesaler's systems stops the drugs moving. Amgen's 10-K says a security breach that impairs the distribution operations of our wholesalers could significantly impair our ability to deliver our products8.
The share of receivables owed by the three, 75% at the end of 2025 against 70% a year earlier9, is the figure that would show the exposure growing. A rise above 80% would mean Amgen's working capital depends even more on three companies' payment cycles.
