ENBREL: From $5.4 Billion to $2.2 BillionThin moat

Amgen (AMGN) — moat facet

ENBREL lost 59% of its sales since 2017 without a single American biosimilar, because Washington and the payers cut its price first.

ENBREL shows what happens to an old biologic in America even without a biosimilar. It sold $5,433 million in 20171 and $2,226 million in 20252, down about 59%3. It was 23.5% of product sales in 2019 and 6.3% in 20254.

A long decline with no biosimilar in sight.

No American copy caused this. The 10-K notes that ENBREL biosimilars are approved in the United States but have not launched, and have launched only in Canada5. Amgen's patents on the fusion protein run to November 2028 and on the DNA and methods of making to April 20296. The fall came from price. In 2025 alone sales fell 33% on a 36% lower net selling price, from 340B mix, the Medicare Part D redesign and higher commercial discounts, while volume rose 4%7.

Then Medicare took over. CMS selected ENBREL for Medicare price setting beginning in 20268, and in the second quarter of 2026 sales were $580 million, down 4%, with a 22% lower net selling price that reflects Medicare Part D price setting under the Inflation Reduction Act, effective January 1, 20269. In the first quarter ENBREL sold only $320 million, down 37%10. The mechanics of the negotiation are covered on AbbVie's and Merck's pages; the Amgen point is how early ENBREL's price had already fallen before the law applied.

ENBREL still earns money and still gains patients. What it no longer provides is pricing power, and it was once a quarter of the company.

The number to follow is ENBREL's quarterly net selling price change. If the 2026 declines reach the 36% of 2025 again, the drug will be worth little by the time its patents expire in 2028 and 2029.

Moat trajectory: Narrowing

Net price down 36% in 2025; Medicare price setting from January 2026.

The price erosion of the oldest franchise; a return to 30%-plus price declines would empty it before the 2028-2029 patent dates.
