Amgen (AMGN) — threat to the moat

Amgen treated more ENBREL patients in 2025 and earned 33% less, largely because a federal discount programme keeps widening.

The 340B drug-pricing programme was designed to let hospitals serving poor patients buy drugs at a discount. Amgen's complaint is about how far it has spread. Its 10-K says the inappropriate expanded utilization of the 340B Program has had, and is expected to continue to have, a negative impact on its business1.

The effect is visible in its oldest large drug. ENBREL's sales fell 33% in 2025, which the 10-K attributes primarily to a lower net selling price of 36% from increased 340B Program mix, the Medicare Part D redesign and higher commercial discounts, partially offset by volume growth of 4%2. More patients were treated and Amgen earned much less on each of them.

This is the kind of discount a drug maker cannot negotiate away. The rebate to a benefit manager is a trade: price for formulary position. A 340B discount is set by statute and grows with the number of pharmacies and clinics that claim it, which Amgen does not control.

Chargebacks, the line where most of these discounts land, rose from $7,090 million in 2019 to $16,988 million in 202534, about 2.4 times5, while net product sales rose about 1.58 times6.

The warning sign would be the same pattern spreading beyond ENBREL. If the next annual report names 340B mix among the reasons for lower prices on Repatha or Prolia, the discount will have become a feature of the whole portfolio rather than of one ageing drug.
