The switching cost is a motor skill held by people Intuitive does not employ, and 3.15 million operations a year keep teaching it.
The switching cost in this business is not a contract. It is a skill, held in the hands of people Intuitive does not employ, and it took twenty-seven years to accumulate.
Operating a da Vinci is a learned motor competence. A surgeon sits at a console, hands below a stereo display, and the system translates finger movements into instrument movements inside the patient, filtering tremor and scaling motion — a three-millimetre hand movement becoming a one-millimetre instrument movement1. Fluency in that takes cases, and cases take years. Around 3,153,000 da Vinci procedures were performed in 20252, each of them building proficiency in this console and no other.
Intuitive has industrialised the teaching of it. The 10-K describes training pathways for surgeons and physicians, residents and fellows, operating-room care teams, patient-side assistants and robotic coordinators; learning engagements including case observation, in-service training, simulation, proctoring and curriculum development; and learning technologies including the SimNow virtual-reality simulator and the Intuitive Learning management system3. Much of it happens at Intuitive's own training centres, taught by Intuitive staff and surgeon instructors.
That apparatus does two things at once. It lowers the barrier to a hospital starting a programme, which is how the installed base grows. And it raises the barrier to that hospital ever leaving, because the asset created is specific — a department full of people fluent in one manufacturer's interface, whose competence would partially reset on another.
The evidence that it works is utilisation. Procedures per system per year rose 3% in 2025 and 3% again in the June 2026 quarter4, on a base growing 12%. A fleet that is both expanding and being used harder is a fleet whose users are getting better at it, and each increment of proficiency is a further reason to stay.
The limit is that a skill is transferable in a way a contract is not. A surgeon who learns a rival console does not forget da Vinci, but they do acquire an option. That is why the competitive threat here is not a better robot; it is a rival that gets into residency programmes.
Judge it on utilisation, +3% a year. Rising utilisation on a growing base is the compounding this facet describes. Flat utilisation with a growing base would mean systems are being placed into programmes that are not developing.
Three million procedures a year keep training the same skill on the same interface, and the installed base of trained surgeons grows with every placement. The skill is no less specific than it was; it is simply no longer the thing that sets the growth rate.
Every recurring dollar is a function of this number. The moat decides Intuitive's share of each operation; this decides how many operations there are, and it is set outside the company.
