A GLP-1 competitor does not take the case to another hospital; it removes the case.
The most unusual competitor in this file does not make a robot, does not sell to hospitals and does not need an operating room. It is a class of drugs.
Intuitive's 10-K records the effect without naming the cause: "The number of U.S. da Vinci bariatric procedures performed declined in the high-single digits in 2025 compared to 2024"1, in a year when total United States procedures grew 15%. On the second-quarter 2026 call management attributed continued pressure on bariatric volumes to GLP-1 obesity medicines2.
This is a different category of threat from a competing platform. A rival robot contests share of a procedure; a GLP-1 receptor agonist removes the procedure. A patient who loses substantial weight on a drug does not have a sleeve gastrectomy at a competitor's hospital — they do not have one at all, and the instruments, the system time and the surgeon's case are all gone with it.
Bariatric surgery is a small share of Intuitive's volume, so the direct impact is modest. The reason it matters more than its size is what it demonstrates: the demand for surgery is not fixed. It is a function of whether a non-surgical alternative exists, and pharmaceutical innovation is now producing those alternatives faster than at any point in Intuitive's history — in obesity today, and in adjacent metabolic and oncology indications that touch other da Vinci categories.
Set against that, the company's counter-argument is real. Management has said the underlying disease burden is unchanged and that deferred procedures will eventually return; obesity drugs also make some patients fit enough for surgery they could not previously have. And weight loss increases the incidence of gallbladder disease, which is a cholecystectomy.
The signal is bariatric procedure growth, which Intuitive discloses annually in its general-surgery commentary. High-single-digit declines while the rest of general surgery grows 18%3 is a category being substituted, not deferred.
GLP-1 medicines are reducing bariatric surgery volumes, and more than half of da Vinci procedures are for benign conditions where a drug is a live alternative. This competitor takes the case away rather than to a rival.
Intuitive says weight-loss drugs have already reduced bariatric procedures, and more than half its cases are for benign conditions. A continued slowdown concentrated in those categories would show the drugs removing operations rather than delaying them.
