Intuitive spent a decade moving onto Ethicon's ground, and Ethicon's owner has now moved onto Intuitive's.
Johnson & Johnson received FDA De Novo authorisation for its Ottava robotic system on 22 July 2026, six months after filing, for a set of general-surgery procedures in the upper abdomen including Roux-en-Y gastric bypass, gastrectomy, cholecystectomy, splenectomy, gastric sleeve, small bowel resection, appendectomy and hiatal hernia repair1. Its architecture is genuinely different: the robot is integrated into the operating table rather than mounted on carts or booms, which J&J says takes 30% to 50% less space.
The competitive significance is not the machine. It is that Johnson & Johnson, through Ethicon, has sold surgical staplers, energy devices and sutures into every operating room in the developed world for decades — and Intuitive has spent the past ten years moving onto exactly that ground.
Intuitive's own 10-K makes the connection explicitly: "as we add new robotically controlled products (e.g., da Vinci stapling and da Vinci energy products) that compete with product offerings traditionally within the domains of open surgery and/or conventional MIS, we face greater competition from larger and well-established companies, such as Johnson & Johnson and Medtronic plc"2. The SureForm and EndoWrist staplers, the Vessel Sealer Extend, SynchroSeal and the E-200 generator3 are all products that displace something Ethicon used to sell.
So this is not a challenger entering a market; it is a supplier of the same operating room deciding to defend it by owning the platform rather than the instrument. And the indications Ottava was cleared for are, almost item for item, the general-surgery categories driving Intuitive's recent growth.
The selected indication list is the thing to watch rather than the placements. Ottava was authorised for cholecystectomy and hiatal hernia repair on day one — two of the four procedures Intuitive named as the largest contributors to United States general surgery growth in 20254.
Ottava's De Novo in July 2026 puts the owner of Ethicon — the company whose laparoscopic instrument business Intuitive spent a decade taking share from — onto Intuitive's own ground, with the surgical sales force already in the building.
Ottava is authorised for general surgery, Intuitive's largest U.S. category. Growth here slowing faster than elsewhere once Ottava ships would show J&J's sales force converting its instrument customers.
