Intuitive Surgical (ISRG) — the future bets

No moonshot — a feature Europe has not certified, a single-port system in its early stages, a digital layer with no price, and four governments deciding whether to pay.

Intuitive's future bets are unusually incremental, and that is a fair description of the company rather than a criticism. There is no adjacent market it is trying to enter and no acquisition strategy; the plan is to make the existing platform better, put it in more countries, and get more procedures onto it.

Lower variance and lower ceiling than most future-bet branches here. Nothing depends on inventing a new business model.

The first is the generational upgrade, which is still in its early innings outside the United States. Da Vinci 5 reached 1,231 installed systems by the end of 2025 and was 870 of the year's 1,721 placements1, but Intuitive is running "a phased launch of our da Vinci 5 surgical system over several quarters" in international markets, and the European certification obtained in July 2025 explicitly excludes force feedback, for which the company will "intend to seek European certification for the use of force feedback in the future"2. The single most differentiated feature of the flagship is not yet approved in Europe.

The second is single-port. Da Vinci SP delivers three multi-jointed instruments and a fully articulating endoscope through one cannula, for single-incision or natural-orifice surgery3. Intuitive describes itself as "in the early stages of launching" it, with 377 systems installed at the end of 2025 and FDA clearance for urologic, colorectal, general thoracoscopic and certain transoral procedures. A measured European rollout began after certification in January 2024.

The third is the digital layer, and it is the one that could change the economics rather than the addressable market. Da Vinci 5 carries more than 10,000 times the computing power of its predecessor and connects to Case Insights, Intuitive Hub, SimNow and the My Intuitive app4; in October 2025 the FDA cleared a software release introducing artificial intelligence "across Ion's entire navigational workflow"5. A company that already sees several million procedures a year has the largest surgical dataset in existence and has barely begun charging for what it knows.

The fourth is geography, and it is the largest number on this page. Outside-the-United-States procedures grew 23% in 2025 against 15% domestically, with particular strength in South Korea and India6, and the company expects OUS to become a greater share of the business over time. Japan expanded reimbursement on 1 June 2026, largest category inguinal hernia, and Japanese placements went from 15 to 25 in a quarter7.

None of these transforms the company. What they do is determine whether procedure growth returns to the high teens after 2026, which is the only question the valuation actually turns on.

Moat trajectory: Widening

Force feedback shipping in the flagship, single port at 377 systems, a digital layer built on the largest surgical dataset in existence, and 23% of revenue outside America with Japan and China both moving. Modest bets, all of them progressing.
