Eli Lilly (LLY) — threat to the moat

A duopoly for now — and duopolies invite everyone else to try.

Lilly does not own the incretin market; it shares it with Novo Nordisk1, a formidable, deeply-experienced rival whose own GLP-1 drugs — semaglutide, sold as Ozempic and Wegovy — created the category and remain among the best-selling medicines in the world. The obesity and diabetes market, for all its size, is fundamentally a two-horse race at the top, and Novo is a competitor with decades of incretin expertise, enormous resources, and every incentive to defend and extend its position against Lilly.

Lilly's obesity brand went from nothing to $13.5 billion in two years against an entrenched rival.

The danger is that this is a war, not a walkover, and its front keeps shifting. Novo and Lilly leapfrog each other on efficacy, race for new indications, compete for manufacturing capacity and payer coverage, and are both pouring resources into next-generation drugs, including oral versions. A rival this capable means Lilly cannot count on its current clinical lead, must keep spending heavily to stay ahead, and faces genuine competition for share, pricing, and the loyalty of prescribers and payers in every market.

Lilly's tirzepatide has, by most measures, the efficacy edge for now, and its pipeline is arguably the strongest in the field — real advantages in the contest. But an owner should recognize that the incretin market, however vast, is intensely contested at the top by a peer as serious as Lilly itself, that leadership will swing back and forth as each company's next drug arrives, and that the enormous profits at stake guarantee the competition with Novo will remain relentless for as long as the market matters.
