So Lilly's first-and-best position is a real and valuable advantage that has let it seize the lead in a generational market — but one it must defend with continued innovation, because in drug development, today's best is always the target of tomorrow's challengers.

Being first built the head start; being best keeps prescribers and payers choosing Lilly. The lead is contestable — Novo is formidable and clinical leads get overtaken — but Lilly is out-innovating to widen it. On balance, a lead being pressed forward, not merely held.
